Death Certificate

Required documents

1. The deceased

  1. Original death confirmation letter issued by the local authority, along with one copy. This document must be certified by a Swiss authority. You can download information about certifying the Swiss death certificate here.
  2. Death confirmation document from the hospital
  3. Funeral arrangement confirmation (e.g., cremation certificate)
  4. Copy of Thai national ID card (Please include the page showing the house registration number and name)
  5. Copy of Thai house registration document
  6. Copy of Thai passport

2. The notifier (Thai or other nationality)

  1. Application form for death registration
  2. Copy of Thai national ID card
  3. Copy of Thai house registration document (Please include the page showing the house registration number and name)
  4. Copy of Thai or foreign passport

Procedure: The notifier must prepare all documents and send the copies along with a contact phone number to the Royal Thai Embassy by post in advance. Please mark the envelope with "Application for Death Certificate". This allows the staff to review and prepare the documents in order to schedule an appointment. Original documents must be presented on the appointment date.
